Timothy A. Lattimore Sr., MSN, APRN, FNP | Co-Founder
Timothy is a family nurse practitioner in Jacksonville, Florida. He currently works as an Occupational Medicine provider for HCA MD-Now Urgent Care. His role is to provide onsite medical coverage for the Jacksonville Electric Authority (JEA). His duties include providing emergency medical and routine care for JEA employees and affiliates. He is a certified medical examiner able to perform preemployment, Fit-for-duty, OSHA and DOT physicals required for employment with JEA. Prior to his role with MD-Now he worked as a primary care provider and specializes in family medicine at Family Practice Associates of Orange Park, Florida. He assessed, diagnosed and treated all health conditions that presented to the clinic. He managed and coordinate care for patients with acute, chronic and complex health conditions, some of which required referrals to see specialist. His patient clientele ranged from school-aged to geriatric.
Timothy is a disabled military veteran who served in the United States Army for 7 ½ years as a medical specialist, emergency medical technician, and practical nurse. He was honorably discharged in 1996. After his time in the service, he obtained a bachelor’s degree in nursing. He now holds a Master’s Degree in Nursing as a Family Nurse Practitioner. He has over 30 years of health care experience. He has a love and passion for providing holistic patient and family centered care grounded in evidence-based practice.
Timothy and wife Stephanie have been married for 25 years. They have four adult children Timothy Jr., Donnielle, Christopher and Skylar. Two grandchildren Journei and Mason. Timothy enjoys spending time with family, fishing, sports and traveling in his leisure time.

Family Practice Associates of Orange Park, 9680 Argyle Forest Blvd. Suite 20. Jacksonville, FL 32222. Family Nurse Practitioner, primary care provider began employment September 2019 until October for 2021. Provided care to patients from infancy to geriatric in the clinic, hospital, or other medical environment following established clinical standards and practices. Worked autonomously as well as collaboratively with the entire clinic team to ensure the delivery of high quality and efficient health care. Duties included but not limited to were, obtain and document complete medical history and physical examination of patients, interpret and integrate data to determine and order appropriate diagnostic and therapeutic procedures or specialty referrals. Manage medical and surgical needs of patients in accordance with scope of practice and established credentialing agreements. Reconcile patient medication and allergies, assess patient’s understanding and address any barriers to medication and therapeutic compliance. Administer medications and injections according to established practice protocols and perform other clinical functions as needed, provide patient education, interview and advise patients regarding health and illness prevention. Recommend community resources to meet patient and family needs, educate patient and family regarding medications and treatment options and corresponding instructions. Participate in after-hours call as required by work setting, maintain complete and accurate medical records in a timely manner, complete patient charge and billing in a timely manner as required by work setting, promote patient satisfaction in all clinical settings.
Orange Park Medical Center, 2001 Kingsley Ave., Orange Park, FL, 32073. Began employment in June 2003, still currently employed as a registered nurse (RN). Previously worked Stepdown Unit and Progressive Care Unit from 2003-2008. Current role Post Anesthesia Care Unit nurse 2008-2019. Currently Charge Nurse (CN) of the Post Anesthesia Care Unit (PACU) since 2016. My current role requires me to administer medications for pain control, moderate sedation, and airway management of patient’s recovering from anesthesia. I assist anesthesia with trauma cases that require mass transfusion of blood product via cell saver or Belmont, peripheral nerve blocks, arterial lines, central lines, epidurals and blood patch procedures. I maintain airway management of patients undergoing electroconvulsive therapy treatment and ventilator management. I provide discharge education to patients and their families and work closely with surgeons and anesthesia providers. I act as a lead nurse during codes. Additional duties include the management of nursing staff, staff evaluations, payroll, scheduling and disciplinary counseling.
St Vincent Hospital Riverside 1 Shircliff Way, Jacksonville, FL 32204. Began employment August 2011 to August 2013 in the Post Anesthesia Care Unit (PACU) as a staff nurse. My role required me to administer medications for pain control, moderate sedation, while monitoring the recovery of patients from anesthesia. It required competency in assisting anesthesia staff with performing peripheral nerve blocks prior to surgery, assisting with epidural placement and blood patch procedures. It required competency in assisting with the placement of central/arterial lines and in managing hemodynamically unstable patients as well as those requiring ventilator management. Other duties included providing discharge education to patients and families and working closely with surgeons and anesthesia providers.
Baptist Hospital Downtown 800 Prudential Dr, Jacksonville, FL 32207. Began part-time employment of September 2007- September 2008 as a Progressive Care Unit (PCU) staff nurse. Job description consisted of performing acute inpatient care specific to patients with acute and chronic disorders such as; hypertension, diabetes, renal failure, chest pain, CAD. Care requirements included performing physical assessments, documentation, medication administration, IV starts, telemetry monitoring and rhythm interpretation, blood transfusions, peritoneal dialysis, pre and post procedure care and wound care/wound VAC dressing changes. Keith A. Lattimore, MBA | Co-Founder
Keith is the Director of Housing Stability with Ramsey County, Minnesota. He has over 25 years’ experience with local government, holding several leadership positions during his Ramsey County career. Keith leads homelessness efforts within Ramsey County and his department serves as the backbone agency to the federally mandated Heading Home Ramsey Continuum of Care (HHRCoC). His department is heavily involved with homeless efforts in Ramsey County, including veteran homelessness, emergency shelter operations, homeless prevention, housing court, family and youth shelters, rapid rehousing, housing support, and operating winter warming space. He relies on community partnerships, government relationships, and people with lived experience to help make homelessness rare, brief, and non-reoccurring in Minnesota.
Keith moved to Minnesota in 1990 from Florida. He attended college and graduated with a bachelor’s degree in criminal justice. He also holds an MBA with emphasis in healthcare administration. Keith has a passion for working with justice involved individuals and people with disabilities. He has lived-experience with disability work, gained through his 22-year-old daughter, Mya. She suffers from spastic quadriplegia; a severe form of cerebral palsy that has required total care since birth.
Keith and wife Dana have been married for 26 years. Together they have 3 kids Mya, Tyler, and Keegan. Keith enjoys traveling with family, fishing with his sons, watching sports, and supporting Team Mya Rose’s tandem running team events. Inclusion is Mandatory!
Keith is the current director of housing stability in Ramsey County Minnesota. (See Link) He has over 25 years’ experience with local government, holding various leadership positions within Ramsey County. His primary responsibility is to provide the overall vision, long-range strategic planning, leadership and direction for Housing Stability, and maintain strong partnerships with a wide variety of stakeholders. Housing Stability serves as the backbone agency to the federally mandated, Heading Home Ramsey Continuum of Care. (HHRCoC) Keith has served as the steering committee’s board chair of the (HHRCoC) and his department plays a critical role making homelessness in Ramsey County, Rare, Brief and Non-reoccurring. His department manages coordinated entry, Family Homeless Prevention and Assistance Program (FHPAP) funding, Rapid Housing, assist Housing Court, Homeless Prevention, operate overnight Winter Warming Spaces, and currently building Ramsey County’s Single-Point of Entry for (HHRCoC). He is responsible for managing contracts between county and community partners. He oversees federal grants, levy funds, state allocations, and philanthropic donations awarded to Ramsey County.
During the COVID pandemic, Keith’s department operated five, 24-hour emergency shelters including a COVID respite program, designed to protect the homelessness network against unnecessary outbreaks. His department served over 737 unsheltered clients throughout the pandemic, using a combination of Coronavirus Aid, Relief, and Economic Security Act (CARES) and American Rescue Plan Act (ARPA), and levy funding.
Media Coverage of Homeless Work:
Keith served as Division Director for MN Choice for and Aging and Disability Services for Ramsey County Social Services Department from January 2019 until redeployed to lead Ramsey County’s homeless work during the COVID pandemic. As division director, he served as a member of the Minnesota Association of County Social Service Administrators (MACSSA) for 2 years. (MACSSA) is a statewide association made up of county public social service directors or other administrative designees. The Association has been in existence since 1946 and is representative of all 87 Minnesota counties. It guides social service policy for the state of Minnesota, setting direction on desired outcomes. It provides guidance to state legislators with a focus on meaningful system improvements.
Keith’s division totaled over 250 professionals, broken down into two units. The MN CHOICE unit was responsible for conducting disability assessments to determine eligibility for federal disability waivers. The unit primarily consisted of registered nurses and social workers, who functioned as certified assessors to conduct initial and annual assessments for the disabled population in Ramsey County. The Aging and Disability Service unit provided case management services to residents on federal disability waivers. Keith was responsible for managing over $500+ million annual dollars and providing case management services to 10,000+ disabled residents in Ramsey County. His responsibility also required him to approve and monitor nearly 100 service contracts, set county policy on disability services, and advocate for legislative improvements to disability services.

Cedric has worked in the service industry for over 30 years, 20 years of service with Ramsey County in the fields of Health & Wellness, Community Corrections (Detention & Probation) and Social Services departments. He currently is working in Withdrawal Management & Detoxification as the Program Manager – Responsibilities include the design, development, and evaluation of (EBP) evidence based practiced services that are received by clients. He ensures that the day-to-day operations of a 50-bed center follows requirements that meet Rule 32 department of human services standards. Also, supports and coordinates nursing, psychopharmacological, psychiatric, and medical services to make sure that they are appropriate treatment plans for clients when needed. Moreover, confirms that the withdrawal management & detoxification facility operates at industry standards. He certifies that all (HIPPA) Health Insurance Portability & Accountability Act and 42 CFR Part 2, (DHS) Department of Human Services licensure requirements along with State policies 9530 & 245F, Ramsey County personnel rules and Union contract obligations are adhered to. Additional experiences appointed to interim assistant superintendent for 10 months where he provided leadership, direction for the detention center assisting the superintendent in the operations of the detention center.
